1. Use the drop-down fields below to select your country and preferred Smile Marketing Plan and then click the "Download Service Agreement" link:

2. Print out the Service Agreement, fill it in and fax it to our US office on +1 (303) 957 2333.

3. On receipt of your faxed paperwork, we email you a link to an online form where your provide practice details and design direction. Your Smile Marketing site will be live, earning its keep, within 30 days!
